TeamHEALTH provide services and support to Territorians who have a mental health concern or are vulnerable and disadvantaged and have fallen through the cracks. We have been doing so for over 30 years.
TeamHEALTH services and supports
TeamHEALTH has developed over time and now provides a range of supports including: Adult and Group Based Support, Children and Families, Residential Support, Housing, Education and Training and Support Coordination.
TeamTALK is a free support service, where any Territorian can quickly access an experienced, locally based mental health recovery professional for a judgement free, confidential chat. You can call, chat online, or even have a video catch up. You don’t need to have a mental illness, either now or in the past, to access TeamTALK. 7 days a week – weekdays 9 AM till 8 PM, and weekends 12 till 8 PM. Call TeamTALK on 1800 832 600
TeamHEALTH Community Hub Community Hub programs are based on structured social, recreational, and educational activities. These are all aimed at supporting living at an optimal level of independence.
Mental Health Supports TeamHEALTH have a variety of supports available for adults aged 16 to 65 who have a mental illness or are experiencing mental health concerns. TeamHEALTH also offer community access and capacity building support to people with or without an NDIS plan.
Two Ways Mentoring Program The Two Ways Mentoring Program is peer lead and aims to increase the economic and community participation of individuals living with mental health concern.
The Way Back Suicide Recovery and Support For people who have attempted suicide, or experienced a suicidal crisis, and who experience severe distress in the days and weeks immediately afterwards and are at high risk of attempting again. A Support Coordinator contacts the person within one business day of receiving their referral and works with them to tailor a program for up to three months after discharge from hospital.
Home Care Packages It is a government subsidised program where an allocation of funds is approved which enables to purchase care services to support people’s independent lifestyle.
Support Coordination TeamHEALTH is a registered provider of Support Coordination services with the National Disability and Insurance Scheme (NDIS). Our Support Coordinators are experienced and skilled in assisting people living with a disability, mental illness, and/or complex needs. Our tailored services empower people to exercise choice and control when identifying the supports and services required to achieve their goals.
Residential Supports TeamHEALTH residential supports provide safe and comfortable accommodation solutions to support you on your journey to recovery and include a range of different options based on your needs.
